Policies and procedures
1. Refund Policy
- Refund Eligibility: Refunds are available if a cancellation request is received at least 48 hours before the scheduled appointment.
- Processing Time: Approved refunds will be processed within 10 business days from the date of the request.
- Non- Refundable Fees: Same-day appointment fees, mileage fees, and any lab processing fees are non- refundable.
- Partial Refunds: If testing has started but not completed, a partial refund may be issued, excluding costs for services already rendered.
2. Additional Parties
- Base Fee Coverage: The base testing fee includes up to two parties (e.g., one alleged father and one child).
- Additional Party Fee: For each additional party (e.g., another child, mother, etc.), an extra fee of $60.00 will be applied.
- Group Discounts: Discounts may be available for testing more that three additional parties. Contact our office for a customized quote.
- Documentation: All parties must provide valid identification and consent forms before testing.
3. Same Day Appointment Fees
- Urgency Fees: A same-day appointment can be arranges for an additional fee of $1,200.00, subject to availability. Urgency Fee must be paid in full at the time of booking.
- Booking Window: Same- day appointments must be booked at least 4 hours in advance.
- Availability: Same- day appointments are not guaranteed and are based on the availability of staff and resources.
4. Mileage Policy
- Standard Mileage Rate: A mileage fee of .23 per mile will be charges for the travel beyond a 25 mile radius from the main office of service provider's location.
- Extended Travel: For distances exceeding 50 miles, additional travel time may be billed at an hourly rate of/////per hour.
- Calculation: Mileage is calculated from the starting point to the appointment location and back to the starting point.
- Waivers: Mileage fees may be waived for bulk testing contracts or under special agreements.
5. Cancellation Policy
- Standard Cancellation: Cancellations must be at least 48 hours before the scheduled appointment to avoid any penalties.
- Late Cancellations: Cancellations made within 24-48 hours of the appointment will incur a cancellation fee of $100.00.
- No - Show Policy: Failure to appear for an appointment without prior cancellation will result in the full charge of the service fee, and no refunds will be issued.
- Rescheduling: Appointments can be rescheduled once without penalty if notice is given at least 24 hours in advance.
